We ask upon the request for any release of health information (medical records), an Authorization for Release of Medical Information be completed.

Electronic Authorization and Submission, please visit this link and the request will be submitted directly to our Release of Information team.
Other Methods, please download the HIPAA compliant authorization and drop off to any CCOC location, mail, fax, or email.

We can also provide a secure electronic option for your imaging through Nucleus.io. Please submit your request through an above means and choose to have your images emailed.

Processing Time
- Patient requests are generally processed by our copy service within 10 business days. Please indicate if you have a pending doctor’s appointment at the time of request to expedite this release.
- Attorney, insurance, legal and copy service requests are generally processed within 28 business days.
- Please allow for additional time if records are being mailed.
Please be advised that a copy fee is assessed in accordance to ORC 3701.741 and updated each February per ORC 3701.742. Please contact the Release of Information department at (330) 670-4076 for additional questions.
